A dental office stamps paper at almost every stage of a visit. The dentist's practice stamp, with name, degree such as DDS, DMD or BDS, the registration or license number where required, address and phone, goes on referral letters, treatment estimates and the lab work orders the dentist signs. Reception stamps appointment and recall cards, marks handwritten receipts PAID and keeps a dated copy of every paper insurance claim. When a crown or denture comes back from the lab, its delivery slip gets a RECEIVED stamp before the case goes to the treatment room.

This dental stamp maker loads an oval logo layout in the editor above, the shape many practices put on appointment cards. Set the practice name in the top arc and the town at the bottom, replace the sample emblem with your own logo, then try a round layout for the dentist's personal details. The same online stamp maker builds the PAID, RECEIVED and recall date stamps for the front desk.

One decision comes before any design: which stamps carry a person's name. The dentist's stamp, with registration details, belongs to that dentist. Associates and hygienists who sign their own documents need stamps of their own, while reception and lab stamps show only the practice. For more layouts for recall and lab dates, browse the date stamp templates.

Stamps a dental office uses

The first carries the dentist's identity. The rest belong to the practice and are shared by reception, nurses and the treatment coordinator.

  • Dentist's practice stamp

    Dr. Priya Raman, DDS · Maple Grove Dental · 75 Cedar Lane, Austin, TX 78704 · (512) 555-0163 · Lic. No. 00000

For referral letters, treatment estimates and lab work orders the dentist signs. Add the license or registration number only where your board or insurers expect it on paper.

  • Appointment card stamp

    YOUR NEXT VISIT · ______ at ______ · Check-up ☐ Hygienist ☐ Treatment ☐ · Please give 24 hours' notice to change

Stamped on the back of business-size cards at checkout. The tick boxes save staff from writing out the type of visit each time.

  • Recall reminder

    RECALL DUE · ______ 2027 · 6 months ☐ 12 months ☐

For the patient's card or the paper chart cover, so reception sees when the next check-up is due without opening the record.

  • Lab case received

    LAB CASE RECEIVED · Date ______ · Case No. ______ · Checked by ______ · Shade OK ☐

On the lab's delivery slip when a crown, bridge or denture arrives. Compare it with the work order before the patient's appointment, not during it.

  • Paid receipt stamp

    PAID · Date ______ · Amount ______ · Cash / Card / Check · Initials ______

For handwritten treatment receipts and payment-plan cards. Invoices printed by the practice software normally need no stamp.

  • Claim copy stamp

    COPY · CLAIM SUBMITTED · Date ______ · Ref. ______

On the office copy of a paper claim or pre-treatment estimate, so anyone can see when it went out and follow it up with the insurer.

  • Children's reward stamp

SUPER BRUSHER · Brave visit today

A cheerful stamp for children's appointment cards and sticker sheets. Stamp paper, not skin, unless the ink is sold as skin-safe.

How to make a dental office stamp

  1. 1

    Decide who signs what

List the documents each person signs: dentists, associates, hygienists, front desk. Personal stamps go only to people who sign under their own registration; everyone else uses practice stamps.

  2. 2

Choose shapes by where they land

Oval or round for cards, rectangles for lab slips and receipts where the stamp has to fit a printed box. Set the plate size first so the wording is fitted to it.

  3. 3

    Leave room for handwriting

Keep the practice name largest and any license number on a straight line. Make blank lines at least 25 mm long, so a date, case number or amount fits in normal handwriting.

  4. 4

    Proof on your card stock

Print the proof, then stamp a test card with the ink you plan to use and rub it after ten seconds. Export a PNG for approval and an SVG for the stamp manufacturer.

Size, ink and card stock

Size
Appointment cards are usually business-card size, about 85–89 × 51–55 mm, so a 40 × 25 mm oval or 38 mm round stamp leaves room to write. Lab and PAID stamps work at about 45 × 18 mm.
Ink color
Blue or black for the dentist's stamp and claim copies, red for PAID so it stands out on a receipt, and any bright color for children's reward stamps.
Smallest text
Keep license numbers at 8 pt or more and nothing on a card below 7 pt. Recall dates are read by patients at a glance, so give them the largest type on the stamp.
Files
A PNG proof is fine for agreeing the wording with the practice owner; manufacturers want an SVG or a vector PDF. Details of the PNG and SVG stamp downloads are on the pricing page.
Card stock
Glossy or laminated appointment cards repel ordinary water-based ink, which then smears in a wallet. Order matte cards with a writable back, or use a quick-dry ink made for coated stock.

Dental stamp questions

What goes on a dentist's practice stamp?

The dentist's name and degree, the practice name, address and phone, and a registration or license number where your board or insurers expect one on paper. Some practices add a website. Leave out specialist titles you are not registered for; dental boards regulate how dentists describe their qualifications.

Does a dental lab work order need the dentist's stamp?

The work order has to come from the dentist and identify them, and in many countries a custom-made crown or denture legally requires a written prescription from a registered dentist. A stamp is a quick way to add the dentist's details, but the dentist completes and signs the order personally.

Where does the COPY stamp go on an insurance claim?

On the copy you keep, with the date and a reference line, not on the original sent to the insurer. Most claims are now filed electronically, so the stamp mainly serves paper estimates, appeals and the few claims that still go by mail.

Can I stamp children's hands after a visit?

Only with ink sold as skin-safe and non-toxic, and only if the parent agrees. Ordinary stamp-pad ink is not made for skin. Many practices prefer a reward card or sticker sheet that the child takes home, which also puts the practice name on the family fridge.

How many stamps does a small dental office need?

Usually four or five: a personal stamp for each dentist who signs documents, a practice address stamp, an appointment or recall stamp, a PAID stamp and a RECEIVED stamp for lab cases. A dater with a word line can combine several of these if counter space is short.
